Health forms are required for transfer students and SAGE students who live on campus. Please complete these steps before classes begin:
- Open FalconLink → Student Health Records (first-aid kit icon)
- Complete all items under Forms
- You may use a physical exam from the last 12 months if you have one or you can download ours below to be completed.
